Seemingly randomly my Apple Watch will start talking to me and asking me questions. I have turned off voice over. What now?

If it is asking questions it seems more likely you have managed to activate Siri (and confuse her). Look in Settings/Siri&Search/Siri Responses and for the purpose of testing, switch to Silent responses. You can always put it back later when you have restored human superiority and control.

OK am trying and will see if that works. Thanks. As i said I can’t test because it just seems to come on randomly.

Be patient, it will reveal whether my guess was right eventually! If not, try to get a look at the screen when it talks, there might be a visible clue at the same time.
